This is the result of a shorted battery pack.
When not quite secured properly in my XPress,
they were allowed to short together.
Much heat, hissing, and smoke later,
 (yes, from the batteries)
the result is shown below.

Make sure your cells can't short out like this.
It's bad. Very bad.
